The core advantage of picosecond lasers lies in their ability to generate a powerful *photoacoustic effect* by utilizing extremely short pulse durations. Simply put, while traditional nanosecond lasers (such as Q-switched lasers) rely on heat to "burn" and shatter pigment particles, picosecond lasers employ mechanical shockwaves to "vibrate" the pigment into fine dust-like particles. Consequently, thermal damage is minimized, and the risk of post-inflammatory hyperpigmentation (rebound darkening) is significantly reduced.
Based on this principle, the four core treatment applications for picosecond lasers in clinical and aesthetic settings are as follows:
1. **Precise Pigment Removal (Endogenous Pigmentary Lesions)**
This is the most routine and high-demand application for picosecond lasers. It targets and destroys melanin accumulations located at various depths within the skin.
* **Superficial Pigmentation:** Freckles, sun spots, and age spots (lentigines). These typically involve rapid recovery and high clearance rates.
* **Deep/Dermal Pigmentation:** Nevus of Ota and Hori's Nevus. These are treated using longer wavelengths (e.g., 1064nm) to penetrate into the dermis and shatter the pigment.
* **Stubborn/Complex Pigmentation:** Melasma. Melasma is highly sensitive to heat; the mechanical effect of picosecond lasers allows for the gentle metabolism of pigment without triggering increased melanocyte activity.
2. **Tattoo Removal (Exogenous Pigmentation)**
Picosecond lasers are currently considered the gold standard for tattoo removal. Traditional nanosecond lasers are often ineffective against certain colored inks (such as green and blue) and carry a higher risk of scarring.
* Picosecond lasers thoroughly shatter tattoo ink particles into minute fragments, facilitating their subsequent phagocytosis and metabolic clearance by macrophages.
* By switching between different wavelengths, the system can specifically target and treat the entire spectrum of tattoo colors (e.g., 1064nm for black and blue; 532nm for red and orange; 755nm for green and blue).
3. **Honeycomb Rejuvenation & Comprehensive Anti-Aging (Skin Texture Improvement)**
When picosecond lasers are utilized in conjunction with array lenses (such as "Honeycomb" or "Micro-lens" arrays), their primary application shifts to skin remodeling and rejuvenation.
* **LIOB Mechanism:** The laser energy is focused within the dermis, triggering a process known as Laser-Induced Optical Breakdown (LIOB). While leaving the epidermis completely intact, it generates countless tiny "micro-cavities" within the subcutaneous tissue.
Effect: In an effort to repair these micro-cavities, the body vigorously synthesizes collagen and elastic fibers. Clinically, this technique is frequently employed to minimize pores, diminish early signs of fine lines, and brighten skin tone.
4. Repair of Acne Scars and Atrophic Scars
This application similarly utilizes the array lens mode, though the energy density is typically increased.
Through a process of physical subcutaneous disruption, it severs the fibrous adhesions situated beneath depressed acne scars and stimulates localized tissue regeneration, thereby leveling out the depressions. Compared to traditional ablative CO2 fractional lasers, the recovery period for picosecond laser treatment is remarkably brief (typically involving only 1–3 days of redness and swelling), allowing patients to resume their normal social activities without interruption.
